What this document is
«Declaração, sob as penas da lei, que não está presente nenhuma das causas de perda de autorização» is a model of the Brazilian Federal Police (Polícia Federal, PF) from its «Declarações e Formulários» section. The declarant states, under penalty of law, that none of the grounds for loss of residence authorization in art. 135 of Decreto 9.199/2017 apply.
The article is printed on the form itself: the authorization is lost (I) when the basis on which it was granted ceases; (II) when residence is authorized on another basis; (III) after an absence from the country of more than two years without justification. The form is in Portuguese, one page, with a warning about art. 299 of the Penal Code.
