What this document is

“แบบคำขอต่ออายุใบอนุญาตขับรถ” is the Thai Department of Land Transport (กรมการขนส่งทางบก, DLT) application to renew a driving licence. It is addressed to the Registrar (นายทะเบียน) of the land transport office.

The form is in Thai only and has two pages. Page one holds the applicant’s details, the type of licence, the attached documents and a declaration of meeting the legal requirements. Page two is entirely for official use (เฉพาะเจ้าหน้าที่): checks, payment and officers’ signatures.

Who files it

  • a holder of a Thai driving licence that is about to expire, including a foreigner;
  • a holder of a temporary (2-year) licence moving on to the next licence term;
  • one form names one licence type (car, motorcycle, public vehicle); if you renew two types, ask the office whether a separate form is needed.

What the form asks for

  • date of the application (Buddhist Era year, พ.ศ.);
  • full name, age, nationality;
  • address: house number, moo, soi, road, tambon, amphoe, province, phone;
  • type of licence to renew;
  • ticks for attached documents: the licence itself (or a substitute) and two 3×4 cm photos if required;
  • signature by hand.

In section 3 the applicant declares meeting the requirements of Section 47 (3) of the Motor Vehicle Act (no repeated penalties for driving offences within the set period) and, for a public vehicle licence, Section 49 (8). The full conditions are printed on page two of the form.

Documents usually brought along

  • the current (or expiring) Thai driving licence;
  • passport with visa and entry stamp, plus copies;
  • proof of address in Thailand — for example a residence certificate from immigration or an embassy, or a work permit;
  • a medical certificate if the office requires one;
  • two 3×4 cm photos if the office does not take a photo on site.

The exact list, deadlines, fees and procedure (tests, training lecture, booking) depend on the licence type and the office — check in advance on the DLT website or with the local land transport office.
