What this document is

The Personal Information Amendment Consent Form is a form of the LTR Visa Unit of Thailand’s Board of Investment (BOI) for Long-Term Resident (LTR) visa applicants. With it, the applicant asks the LTR Visa Unit to upload their LTR visa application and supporting documents, submitted to ltr@boi.go.th, into the LTR Visa Application System on their behalf.
