What this document is

“Declaración de extravío” is an official form of Spain’s Directorate-General for Traffic (DGT): a written statement that the vehicle’s documents have been lost.

It is attached to the procedure that requires the lost document — for example, a duplicate registration certificate (permiso de circulación), a change of owner or deregistration. The form is in Spanish, one page; FoxDocs fills in the official PDF without changing its layout.

Who files it and where

  • it is made by the person filing the vehicle procedure — usually the registered owner (titular);
  • the form is signed by hand and attached to the procedure at the Jefatura Provincial de Tráfico; in-person service is by appointment (cita previa) on the DGT website or by calling 060;
  • a duplicate permiso de circulación after loss carries fee 4.4 of €20.81 (DGT, as of 2026-10-09); the duplicate itself can also be requested online, by phone on 060 or at a Correos post office;
  • check the current requirements for your procedure on the DGT website.

What the form asks for

  • the declarant’s full name and DNI or NIE;
  • the vehicle’s registration number (matrícula) and make;
  • the procedure that required the lost documents and the province of the Jefatura de Tráfico;
  • place and date; the signature is handwritten.

  • the form of the procedure itself (for example Mod. 01 for a duplicate) and proof of identity: DNI or NIE (TIE, passport with NIE);
  • the receipt for the procedure fee, if one applies;

Frequently asked questions

When is this form needed?

When a procedure at the Jefatura de Tráfico requires a vehicle document (such as the permiso de circulación) that has been lost. The declaration is attached to that procedure.

What does the declaration replace?

It explains why you are not presenting the vehicle document. It does not restore the document: a new one requires the duplicate procedure.

How much is a duplicate permiso de circulación?

According to the DGT as of 2026-10-09, fee 4.4 of €20.81. Requesting it through Correos adds a service charge.

Can I get a duplicate without visiting the Jefatura?

Yes: the DGT accepts duplicate requests online (Cl@ve, electronic DNI or certificate), in the miDGT app, by phone on 060 and at Correos offices.

Who signs the declaration?

The person filing the procedure, usually the vehicle owner. It is signed by hand on the printed form.

Can I use my passport number?

The form says “DNI / NIE” and has no space for a passport. A foreigner enters their NIE (shown on the TIE or on the NIE certificate).
