What this document is

“Declaração, sob as penas da lei, que a Carteira de Registro Nacional Migratório foi extraviada” is a template of Brazil’s Federal Police (PF). The declarant states, under penalty of law, that the National Migration Registry card (CRNM) issued under their previous migration status has been lost.

The form is in Portuguese, one page; at the bottom it warns about Article 299 of the Brazilian Penal Code (false statements in a document).
