What this document is

“Declaração … no último ano” is a template of Brazil’s Federal Police (PF). The declarant states, under penalty of law, that in the last year they had no police inquiry, criminal case or conviction — in Brazil or abroad.

The form is in Portuguese, one page; at the bottom it warns about Article 299 of the Brazilian Penal Code (false statements in a document).

Do I have to give my father’s name?

The form has the line “filho de … e de …”. If your father is not listed on your birth certificate, leave his line blank; write your mother’s name as on the certificate.

Which language do I fill it in?
