“Declaração, sob as penas da lei, de ausência de antecedentes internacionais penais ou policiais” is a template of Brazil’s Federal Police (PF) from its “Declarações e Formulários” section. The declarant states, under penalty of law, that they are not and have never been the subject of a police inquiry (inquérito policial) or criminal case and have not been convicted — in Brazil or abroad, up to the date of signing.

Do I have to give my father’s name?

The form has the line “filho de … e de …”. If your father is not listed on your birth certificate, leave his line blank; write your mother’s name as on the certificate.

Which language do I fill it in?
